For may individuals fat is accumulated in may areas of the body that is unresponsive to even the most stringent diets and exercise and work out routines. This is because diet and exercise decreases the amount of fat in the fat cells and the size of the fat cells. The number of fat cells however remains unchanged. Liposuction is the cosmetic surgical procedure that removes fat cells from these areas and improves body contour and proportion.
This adiposity often is localized and results in a disproportionate body shape and contour. These areas often include the hips\flanks, abdomen, thighs, knees, buttocks, arms, back, and neck.
Liposuction started from the work of surgeons in Europe in the late 1960's. The major advance that Liposuction introduced was the ability to remove fat and contour parts of the body through small skin incisions. Prior to Liposuction the only was to remove and contour adiposity was with a large incision often placed directly over the area of adiposity with a conspicuous scar.
Modern Liposuction began with the presentation of the technique of Dr. Yves-Gerard Illouz in 1982. He infused the areas of adiposity for Liposuction with fluid containing epinephrine, and used blunt cannulas and high vacuum suction. At that time I had the unique opportunity of traveling to Paris and learning the technique of Liposuction directly from Dr. Illouz at his surgical center.
What Liposuction Will Not Improve:
For areas of a significant amount of fat removal the procedure is performed with general anesthesia. For limited small areas of fat removal this may be done with local and or sedation anesthesia.
Sterile fluid containing a vasoconstrictor(epinephrine) is infused into the areas of adiposity to be removed.. A cannula or blunt tube with one or more hole on the end is inserted into the area of adiposity. High vacuum suction is applied to the cannula and the fat is suctioned from the tissue.
This is the same as Tumescent Liposuction except that a greater amount of sterile fluid is into the areas of adiposity. The amount of fluid infused is about the same amount as the volume of fat that is expected to be removed. This method usually produces a better fluid balance. This method also improves the ease of fat removal. The fluid that is infused fills and swells the fat cells. Like a balloon that is overfilled with water and easier to burst that a half filled balloon, it is also easier to burst and suction these distended fat cells.
Fluid is initially infused into the areas of adiposity as in the Super-Wet Liposuction technique. A blunt ultrasonic probe that attached to suction is inserted into the area of adiposity. The ultrasound probe emits continuous high frequency sound waves. These high energy sound waves hit the distended fat cells and break these apart. The liquefied fat is then removed with suction. Since this technique targets the fat cells it is less traumatic to the surrounding tissue than the other Liposuction techniques. This results in less bleeding, less bruising and less swelling.
The energy of the continuous ultra-sound produces heat. The heat that is transmitted through the tissue causes contraction of fibers that attach the skin to the muscle. The contraction of these fibers results in an improved re-draping of the skin after removal of the adiposity. The provides for a smoother and more even contour than other methods.
This is the same as Ultra-Sound Assisted Liposuction (UAL) except that instead of continuous ultrasound waves, the sound waves are cycled on and off, i.e. they are pulsed. The pulsed ultra-sound waves have less heat generation than the continuous ultra-sound wave. This minimizes the possible problem of excessive heat causing burn injury to the skin. A second advantage of a pulsed sound wave is that it causes the wall of the distended fat cell to vibrate back and forth. This enables the sound wave to burst the fat cell easier and with less energy. Pulsed Ultra-Sound Liposuction is the procedure used by Dr. Williams.
Pulsed Ultrasound Assisted Liposuction has several advantages over other forms of Liposuction:
Small soft drainage tubes are placed into the areas of Liposuction to drain the remaining fluid that had been infused, any remaining liquified fat, and blood. Drains speed the recovery process by decreasing the amount of swelling and bruising. Compared to the Tumescent Liposuction the Pulsed ultra Sound Liposuction results in very minimal bruising.
You will be placed in a tight fitting elasticized garment in surgery. This is to decrease swelling and re-drape the skin and help shaping your new contour. This is usually worn for about a month or so.
The amount of pain and the length of time of recovery is dependent upon the amount of adiposity removed and the number of areas treated. Most patients return to work in 7-14 days.
You will also be given Hivamat treatments. These are given once the drains have been removed. This unique state of the art treatment dramatically decreases swelling and bruising. This accelerates the healing process and your slimmer and improved body contour.
Developed in Germany, Hivamat also known as Deep Oscillation Therapy, generates low frequency gentle electrostatic impulses deep in the patient’s tissue. This creates “kneading” of the deep tissue that compresses and pumps lymph fluid and inflammatory by-products into the lymphatic system. The accelerated reduction of edema and inflamation increases the speed of tissue repair.
